SUPPORT COORDINATION

Respite will give both the disability and family to have a short break and release from exhausted and tried both physically and mentally.

In a new place where you can feel safe, home and live, independently on your terms, where your choices and privacy are respected in this for short term break.

A friendly staff member will be supported to live as independently as possible during this short period. You can prepare your own meals with assistance, take part in domestic tasks and even do your own shopping.

There will be a variety of supervised indoor and outdoor activities from shopping to movie nights, and sport. You will have an opportunities to try new things to develop your new skills and have good experiences.

Our duties are to achieve your goals and live as independently as possible.

Our participants the chance to live dignified and happy lives through ongoing reviews of support plans.

We give our participants and their families the resources they need to be able to continually advocate for themselves.

Our support coordinators will monitor the delivery of the services our clients receive, and ensure they remain within the budget also fulfill the goal of participants.

  • accessing community events and activities of your choice, including transportation.

In this supported independent living involve shared accommodation or individual where participant will receive support for a range of activities that may include: laundry, cooking, cleaning, showering, dressing, taking medication, participating in a hobby, using public transport, and attending appointments. The level of support for each of these activities may vary between each participant, depending on the level of support they require.

This could be either temporary or ongoing, with a focus on developing the skills of each participant to live as autonomously as possible.
